Where's the Zoom link?

1. Patient portal:
Log into your Patient portal above and click the "Appointments" tab.
"Join Video Call" link appears on the day of the appointment.

2. Email:
Subject: "Dr. Hwang - Appointment confirmation + Zoom link"
Sender: app@intakeq.com
Sent: The day of our last appointment

3. Text:

Reminder SMS sent the day before your appointment includes the Zoom link.

Prescriptions

I'm in between appointments and need a refill / I didn't get a text or call from my pharmacy saying it's ready.
First call your pharmacy and speak with someone to see if you have active prescriptions on hold (that they haven't filled yet). The pharmacy may need additional insurance information from you prior to filling the medication.
I typically send enough prescriptions/refills to last you until your next appointment and so the med is often on hold at the pharmacy.

 
When will my prescription be ready?
If we discussed sending a prescription at your appointment, I send that prescription that same day. Pharmacies typically take 3-4 hours to process and fill a prescription. You can reach out to them to attempt to expedite the order if needed.
My pharmacy says I need a prior authorization.
Ask the pharmacy for your "RxBin", "RxPCN number", and "RxGroup" on your Drug insurance ID card. Send this to me and I can complete the prior authorization.

    (If using the app, enable "Automatic Claim Status Updates" for the best experience)

No Surprises Act:
All prospective and current patients have access to my service fees by scheduling a phone call or requesting to book. None of my patients will ever receive surprise or unexpected bills from me that are not mutually agreed upon in our payment agreement.

In compliance with Section 2799B-6 of the Public Health Service Act, I send a detailed formal copy of my fees and estimated treatment frequency ("good faith estimate") to all patients who schedule with me and do not plan to submit superbills to their insurance companies. All patients have the right to request and receive Good Faith Estimate paperwork at any time before scheduling or during treatment. If you receive a bill over the price of your Good Faith Estimate, you may start a dispute resolution process with the US Department of Health and Human Services. Visit cms.gov/nosurprises for more information about this act.
